Health Cards: Renewals and Address Changes

The information in this document is aimed to guide you through the process of Renewing or Re-Registering your Ontario Health Card. Please navigate to the section that best describes your situation:

Do you need to renew your OHIP Photo Identification Card?

Please follow these steps to renew your OHIP card.

  1. Print & Fill out the Health Card Renewal form.
  2. Please ensure you have an original document from each of the 3 lists on the Ontario Health Coverage Document List
  3. Bring your completed form and identification documents into the nearest ServiceOntario Health Card Services – OHIP Office. Find an appropriate ServiceOntario Office.

If you are renewing for a child

  1. Print and fill out the Health Card Renewal – Child Form
  2. Please ensure you have an original document from List 2 and List 3 (cannot be the same) on the Ontario Health Coverage Document List
  3. Bring your completed form and identification documents into the nearest ServiceOntario Health Card Services – OHIP Office. Find and appropriate ServiceOntario Office.

Are you switching from the old red and white card to the new health card with photo identification?

  1. Print & Fill out the Health Card Re-Registration Form
  2. Please ensure you have an original document from each of the 3 lists on the Ontario Health Coverage Document List
  3. Bring your completed form and identification documents into the nearest ServiceOntario Health Card Services – OHIP Office. Find an appropriate ServiceOntario office.

Are you moving to a new address?

Don’t take risks with your health coverage. If you are moving you must notify ServiceOntario. There are two ways this can be done

Option 1: Mail in Form

  1. Print and fill out the What’s your address form
  2. Mail completed form to
    ServiceOntario PO Box 48 Kingston ON K7L 5J3

Option 2: ServiceOntario Kiosk

  1. Go to a ServiceOntario kiosk. Find one here.

Option 3: Update Address Online

  1. Update your information online.
