Booking an appointment

To make an appointment to see your family doctor, please call the office at 416-964-0800. If there is no answer, leave a message.  

Please note the following information:

• You must bring a valid healthcard to your appointment, or you will be billed on a fee-for-service basis.
• We book appointments on a priority basis, so if you have an immediate problem, please inform us.
• Depending on availability physicals are usually booked atleast a month in advance.

Cancellations

You must give atleast 24hrs notice if you are unable to make your appointment. We reserve the right to bill you directly for any missed appointments
